Faculty physicians at academic health centers are increasingly interested in incorporating the topics of cultural diversity and healthcare disparities into experiential education activities; however, examples have not been readily available. In this report, we describe a variety of experiential education models that were developed to improve resident and fellow physician understanding of cultural diversity and healthcare disparities. To evaluate the modulation of fundamental frequency (pitch) in tracheoesophageal (TE) speakers following total laryngectomy and voice restoration, 1 l TE speakers completed a series of vocal tasks under standardized conditions. All patients underwent standard laryngectomy with appropriate neck dissection and fullcourse radiation therapy. Each speaker produced four sustained phonations at distinct levels. Sound pressure level (intensity), fundamental frequency, pitch perturbation (jitter), and intensity perturbation (shimmer) measurements were taken for each level. A multiple regression analysis program to assess the relative effect of several variables on predicting fundamental frequency demonstrated that only sound pressure level provided a statistically significant effect. This article describes these trends as well as specific efforts underway in Minnesota that could serve as models for providers in other parts of the United States.

Coordination and collaboration between organizations interested in promoting the health of the po... more Coordination and collaboration between organizations interested in promoting the health of the populations they serve can potentially help to ensure that key services are provided as well as augment the efforts beyond that which could be accomplished by each organization alone. Understanding the perspectives of each organization can facilitate development of health promotion initiatives that will be of mutual benefit. In Maryland, when a Medicaid managed care program was initiated, Memoranda of Understanding were signed between each managed care organization (MCO) and each of the 24 local health departments; many stipulated that the parties will coordinate on community health issues. This report describes a telephone survey of the health departments that was performed by one MCO to better understand the interests and expectations of the health departments and discusses a process for developing a community health promotion agenda for an MCO.

In contrast to the relative frequency of granular cell tumors (GCT) in the larynx and bronchi, th... more In contrast to the relative frequency of granular cell tumors (GCT) in the larynx and bronchi, the occurrence of these tumors in the trachea is rare. A 50-year review of the English-language literature disclosed only 24 described cases of tracheal GCT. This report reviews the clinicopathologic data from those 24 cases, along with the data from 2 cases obtained via a personal communication and the data from 4 previously unpublished cases obtained from a 30-year review of the Armed Forces Institute of Pathology archives. Tracheal resection was the predominant mode of therapy and often was performed as a salvage procedure for failed endoscopic excisions. Recommendations for a more uniform approach to surgical management are provided.

The three trapezius myocutaneous flaps remain valuable adjuncts in head and neck reconstructive s... more The three trapezius myocutaneous flaps remain valuable adjuncts in head and neck reconstructive surgery. Overall, the lower vertical trapezius flap has the most versatility and reliability, and the least morbidity. There are several advantages of this flap compared to the pectoralis myocutaneous (PMC) flap. It has a thin vascular pedicle, and thus does not add excessive bulk to the neck, as the PMC can. Also, the skin island has less subcutaneous tissue and therefore more pliability than the PMC, which is a definite advantage for oral cavity and oropharyngeal defects. The trapezius flap generally has less hair than the PMC flap, and the donor scar is better hidden.

Coordination and collaboration between organizations interested in promoting the health of the po... more Coordination and collaboration between organizations interested in promoting the health of the populations they serve can potentially help to ensure that key services are provided as well as augment the efforts beyond that which could be accomplished by each organization alone. Understanding the perspectives of each organization can facilitate development of health promotion initiatives that will be of mutual benefit. A 50-year review of the English-language literature disclosed only 24 described cases of tracheal GCT. The three trapezius myocutaneous flaps remain valuable adjuncts in head and neck reconstructive s... more The three trapezius myocutaneous flaps remain valuable adjuncts in head and neck reconstructive surgery. Overall, the lower vertical trapezius flap has the most versatility and reliability, and the least morbidity.
